The cases are adding up for the new Independent Investigations Office.
The latest case involves a police involved-death in Vancouver.
Plice say at 10:30 this morning they were dealing with a man who threatened to hurt himself with a knife.
They used a taser to take him down.
The new investigations office says the man hurt himself, was rushed to hospital, but died from his injuries.
